Adult Community Health Nurse

NHS National Services Scotland is seeking an experienced registered nurse to join our team as a community band 5 staff nurse on a fixed term contract for 1 year.
Key Responsibilities:
- Assess and deliver an agreed plan of care to individuals within a community setting.
- Work within the Primary health Care team and under the direction and supervision of the District Nurse.
- Develop and maintain effective relationships with patients, families, and carers.
- Participate in the development and implementation of care plans.
- Contribute to the evaluation and improvement of care services.
Requirements:
- Significant nursing experience and demonstrable leadership and communication skills.
- Evidence of continuing development and a passion for a career in community services.
- Ability to work autonomously within a dynamic environment.
- Current clean driving licence.

NHS Scotland is reducing their full time working week from 37 hours to 36 hours.
This reduction will also be applied pro rata for part time staff.
This advert and any subsequent offer/contract of employment therefore reflects the new working hours.
However, as not all service areas will be able to adopt the 36 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
