Job Title: Community Staff Nurse - Adult Nursing Services

We are seeking an experienced registered nurse to join our team as a community band 5 staff nurse within our out of hours service. The successful candidate will work within the Primary Health Care team and under the direction and supervision of the District Nurse, who will assess and deliver an agreed plan of care to individuals within a community setting.

Key Responsibilities:
  • Deliver high-quality nursing care to individuals within a community setting
  • Work autonomously within a dynamic environment as part of the wider HSCP
  • Develop and maintain effective relationships with patients, families, and other healthcare professionals
  • Contribute to the development and implementation of care plans
  • Participate in ongoing professional development and continuing education
Requirements:
  • Significant nursing experience
  • Demonstrable leadership and communication skills
  • Evidence of continuing development
  • Current driving license
