About the Role

NHS Greater Glasgow and Clyde is seeking a skilled and dedicated Community Staff Nurse to join our team. As a Community Staff Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals in their own homes and communities.

Key Responsibilities
  • Provide skilled clinical interventions to patients aged 5 years and over, encompassing a broad spectrum of technical expertise, health education, and health promotion.
  • Work in partnership with other members of the primary healthcare team and service users to deliver effective and culturally sensitive care.
  • Respond to both planned and unplanned care needs, prioritizing patient care and ensuring timely interventions.
  • Collaborate with GPs, other primary care team members, and secondary care colleagues to ensure seamless care coordination.
  • Contribute to the development and implementation of care plans, ensuring patient-centered and person-centered approaches.
Requirements
  • Hold a live NMC UK pin and be registered with the Nursing and Midwifery Council.
  • Have a strong understanding of community nursing principles and practices.
  • Be able to work independently and as part of a team, demonstrating excellent communication and interpersonal skills.
  • Be able to prioritize tasks, manage time effectively, and maintain accurate records.
  • Be committed to ongoing professional development and continuing education.
