Clinical Care Planner

We are seeking an experienced Paediatric Nurse to join our team as a Clinical Care Planner. In this role, you will be responsible for developing and implementing high-quality care plans that meet the unique needs of paediatric patients.

The successful candidate will have the opportunity to work closely with our multidisciplinary team to provide comprehensive support and guidance to clients and their families.

Key Responsibilities:
  • Designing and implementing individualised care strategies that address the physical and psychological needs of clients
  • Collaborating with the multidisciplinary team to ensure seamless care delivery and optimal patient outcomes
  • Providing ongoing monitoring, assessment, and evaluation of clients' needs, making adjustments to care plans as necessary
  • Establishing and maintaining strong relationships with clients and family members
  • Working collaboratively with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke care packages
Requirements:
  • RSCN with a minimum of 2 years' experience in the UK and an active NMC PIN
  • A supportive management style that fosters growth and development among team members
  • Experience with tracheostomy care and ventilators
  • Advanced communication skills, both verbal and written, to liaise effectively with patients, families, and the multidisciplinary team
  • A full UK driving licence is essential
