Nurse Care Manager Role for Kind and Compassionate Professionals

About the Role: We are seeking a highly skilled Registered Nurse to join our team at Barchester Healthcare, where you will have the opportunity to provide exceptional care to our residents. As a Registered Nurse, you will be responsible for delivering high-quality nursing care, working closely with our multidisciplinary teams to develop and implement individualized care plans that meet the unique needs of each resident.
Responsibilities:
- Deliver safe and effective care to residents, ensuring their physical, psychological, and social needs are met.
- Develop and implement comprehensive care plans in collaboration with residents, families, and healthcare professionals.
- Maintain accurate and up-to-date records of resident care, including medical history, treatment plans, and progress notes.
- Communicate effectively with residents, families, and colleagues to ensure seamless care coordination and timely interventions.
- Participate in quality improvement initiatives, contributing to the continuous enhancement of our care services.
Requirements:
- Current registration with the Nursing and Midwifery Council (NMC).
- Degree in Nursing or related field.
- Experience in caring for older adults or individuals with complex needs.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
