About the Role

NHS Sussex ICB is seeking a highly skilled and experienced Senior Practitioner to join our Clinical Bank team. As a key member of our team, you will play a vital role in delivering an effective, efficient, and equitable service to the people and populations of Sussex.

Key Responsibilities
  • Assessment: Conduct thorough assessments of patients' needs, identifying and prioritizing their requirements.
  • Reviews: Lead and participate in reviews of patients' care plans, ensuring they are up-to-date and meet the individual's needs.
  • Care Planning: Develop and implement comprehensive care plans, working closely with patients, families, and carers.
  • Case Management: Manage complex cases, ensuring timely and effective interventions to meet patients' needs.
Requirements
  • Experience: Proven experience in a health or care environment, with a good understanding of the health and social care environment and roles and responsibilities within it.
  • Skills and Knowledge: Strong assessment and care planning skills, with the ability to manage complex cases and work effectively with patients, families, and carers.
  • Qualifications: Educated to degree level or relevant experience in Nursing, with subsequent up-to-date registration with the Nursing & Midwifery Council (NMC).
