About the Role

We are seeking an experienced Senior Practitioner to join our Clinical Bank at NHS Sussex ICB. In this role, you will play a key part in delivering an effective, efficient, and equitable service to the people and populations of Sussex.

The Senior Practitioner will work within the remit of the National Framework for Continuing Healthcare, delivering a variety of functions across the CHC service, including assessment, reviews, care planning, and case management.

You will also have the opportunity to work across other areas of CHC, including audit, appeals, retrospective assessments, complaints, assessment verification, and many more.

The post holder will be supported to develop the necessary skills and experience to work across the whole pathway, safely, confidently, and effectively.

Key Responsibilities

  • Deliver assessment, reviews, care planning, and case management
  • Work across other areas of CHC, including audit, appeals, retrospective assessments, complaints, assessment verification
  • Develop skills and experience to work across the whole pathway

Requirements

  • Experience of working in a health or care environment
  • Good understanding of the health and social care environment and roles and responsibilities within it
  • Negotiation, conflict management, and influencing skills
