About the Role

We are seeking a highly skilled and experienced Senior Practitioner to join our Clinical Bank team at NHS Sussex ICB. As a Senior Practitioner, you will play a key role in delivering an effective, efficient, and equitable service to the people and populations of Sussex.

Key Responsibilities
  • Assessment: Conduct comprehensive assessments of patients' needs and develop personalized care plans.
  • Reviews: Conduct regular reviews of patients' care plans and make adjustments as necessary.
  • Care Planning: Develop and implement care plans that meet the needs of patients and their families.
  • Case Management: Manage complex cases and coordinate care with other healthcare professionals.
Requirements

To be successful in this role, you will need:

  • Experience of working in a health or care environment, including acute, community, mental health, and local authority settings.
  • A good understanding of the health and social care environment and roles and responsibilities within it.
  • Negotiation, conflict management, and influencing skills.
  • Involvement in operational processes and evidence of using a problem-solving and forward-thinking approach.
