About the Role

NHS Sussex ICB is seeking a highly skilled and experienced Senior Practitioner to join our Clinical Bank team. As a Senior Practitioner, you will play a key role in delivering an effective, efficient, and equitable service to the people and populations of Sussex.

Key Responsibilities

  • Assessment: Conduct comprehensive assessments of patients' physical and mental health needs, identifying areas for improvement and developing personalized care plans.
  • Reviews: Lead reviews of patients' care plans, ensuring they are up-to-date and aligned with their changing needs.
  • Care Planning: Develop and implement care plans that meet the needs of patients, working closely with multidisciplinary teams.
  • Case Management: Manage complex cases, ensuring timely and effective interventions to meet patients' needs.

Requirements

  • Experience of working in a health or care environment, including acute, community, mental health, and local authority settings.
  • Good understanding of the health and social care environment, including roles and responsibilities within it.
  • Negotiation, conflict management, and influencing skills.
  • Involvement in operational processes, with evidence of using a problem-solving and forward-thinking approach.

Desirable Qualifications and Skills

  • Knowledge and experience of working with Safeguarding issues and Policy guidance.
  • Knowledge of the Mental Capacity Act and Best Interests decision making.
  • IT literate, with working knowledge of Microsoft Office packages and intermediate keyboard skills.
