Primary Job Responsibilities- Undertake thorough intake assessments for new inmates to identify potential health risks and devise preliminary crisis management plans.
- Conduct comprehensive health assessments for all willing inmates within seven days.
- Lead the process of identifying patients who need reassessment after returning from court or hospital.
- Collect vital healthcare information from families and community service providers to enhance patient care continuity.
- Deliver timely interventions and contribute to performance targets.
- Provide short-term care coordination and offer brief interventions for substance misuse and mental wellbeing.
- Degree accompanied by a diploma and specialized training.
- Significant clinical experience, encompassing individual and group work promoting social inclusion.
- Documentation verifying eligibility to work, including ID and residency confirmation.
