Clinical Administrator for Proactive Care Team

Job Summary
The Clinical Administrator will provide administrative support to the Proactive Care Team, who deliver proactive clinical support to vulnerable patients and those with complex physical or mental health needs in the Winchester and Andover areas. The aim is to improve their quality of life and healthcare experience, reducing the risk of emergency hospital admission.
This role is a fixed-term contract for 12 months, providing administrative support to the clinical team.
Main Duties
The Clinical Administrator will be responsible for:
Managing and triaging incoming referrals into the Proactive Care Team.
Booking and planning clinical appointments for home visits.
Booking appointments onto EMIS and updating clinical notes.
Preparing documentation for Nurse Appointments, including blood forms, assessment templates, patient details, and surveys.
Completing onward community referrals as required.
Providing advice and signposting to service users, carers, and professionals.
Coordinating clinical attendance at Practice White Board Meetings.
Supporting the clinical team with administrative tasks, meeting minutes, and audits.
Ensuring allocated patients can access services available in the community.
Reviewing elderly and complex patients post-discharge notifications to identify those requiring community follow-up.
Ensuring the clinical team adheres to the Lone Worker Policy.
