About the Role

This is an excellent opportunity for a skilled professional to join Polaris Community as a Children & Young People Mentor. The successful candidate will be responsible for providing specialist support to children and young people in their home environment, helping them navigate transitions, and advocating for their needs within multi-agency networks.

You will work collaboratively with colleagues, birth parents, carers, and other stakeholders to ensure that the highest level of care and support is provided to those in need. To excel in this role, you must possess strong analytical and problem-solving skills, be able to think creatively, and maintain accurate records.
