About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at HC One. As a key member of our care delivery team, you will be responsible for delivering high-quality care to our residents, working closely with colleagues to ensure seamless service delivery.

Key Responsibilities:

  • Develop and implement personalised care plans that cater to the unique needs of each resident.
  • Collaborate with multidisciplinary teams to ensure effective communication and coordination of care.
  • Lead by example, promoting a culture of kindness, respect, and compassion in all interactions.
  • Maintain accurate records and reports, ensuring compliance with regulatory requirements.

Requirements:

  • Possess a relevant S/NVQ 2 or 3 Diploma in Health & Social Care.
  • Have experience in person-centered care planning and a background in caring for individuals with dementia.
  • Demonstrate excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and colleagues.
  • Show commitment to continuous learning and professional development, embracing new ideas and best practices.
