Child and Family Services Coordinator

Polaris Children's Services is a leading provider of children's services, driven by a commitment to innovation and excellence. We're seeking an experienced Family Support Worker to join our team in Nottingham, Nottinghamshire, or Derby.
The successful candidate will have experience working with children and families, as well as a passion for delivering high-quality services that meet the unique needs of each individual.
Key Responsibilities:
- Develop and implement therapeutic programs to support children and young people in transitioning from residential to fostering placements.
- Build strong relationships with families through home visits and community outreach.
- Collaborate with our multidisciplinary team to ensure seamless transitions and support.
- Monitor and evaluate program effectiveness to inform future service improvements.
Estimated Salary: £26,500 - £31,000 per annum.
