About the Role

We are seeking an enthusiastic practitioner to join our Community Mental Health Team in Bridgwater, Somerset. As a key member of our team, you will play a vital role in providing mental health advice and support to adults in need.

Job Description
  • You will manage a caseload of patients, monitoring their mental state, promoting independence, social inclusion, and reducing hospital admissions.
  • Collaboration with Operational Team Managers, Clinical Leads/Practitioners, and community teams is essential.
  • A full driving license and access to your own transport for patient visits are required.
