Job Overview

We are seeking a highly skilled and experienced Senior District Nurse to lead our Community Nursing Teams within the East North Integrated Care Hub. As a key member of our team, you will be responsible for providing senior support and clinical oversight to our teams of highly motivated professionals.

The ideal candidate will have a strong background in community nursing, with a District Nursing Specialist Practitioner Qualification and a UK driving license. You will have the ability to work independently and as part of a team, with excellent communication and leadership skills.

Main Responsibilities
  • Lead and manage Community Nursing Teams within the East North Integrated Care Hub, ensuring high-quality care is delivered to patients in a variety of community settings.
  • Line manage District Nurses/Team Leads, providing guidance and support to ensure the effective management of teams within the locality hub.
  • Promote effective teamwork within the wider primary health care team, delegating activities appropriately to benefit patient care and service delivery.
  • Monitor the quality of care provision, identifying areas for service development and promoting best practices.
  • Oversee the performance of teams and individuals within the hub, ensuring high standards are maintained.
  • Act as a clinical role model, supporting care delivery for complex patients on the Community Nursing caseloads.
  • Work in partnership with key stakeholders to ensure seamless care is delivered, meeting the requirements of commissioners.
  • Act as a key point of contact for GPs and other general practice staff at a hub level.
