**Job Description:**
  • This is a full-time position, working 40 hours per week.
  • You will work closely with service users, their families, and other stakeholders to develop and implement comprehensive social care plans.
**Key Responsibilities and Accountabilities:**
  • Provide expert guidance and support to service users, their families, and other stakeholders.
  • Forge effective relationships with other statutory agencies to promote better outcomes for our service users.
  • Take a lead role in social care planning and delivery, ensuring individual needs are met.
  • Ensure service users and their families/carers are aware of their entitlements and available resources.
  • Contribute significantly to safeguarding efforts within our organization.
  • Promote equality and diversity principles throughout all aspects of your work.
**Requirements:**
  • A Registered Social Worker with Social Work England registration.
  • A relevant Social Work qualification.
  • Experience working within a mental health setting or a passion for this field.
  • Excellent communication and interpersonal skills.
  • Able to work effectively as part of a multidisciplinary team.
