Job Overview:

Responsibilities:
- Deliver care and support services to adults with care and support needs, their families, and carers.
- Develop and implement personalized care plans, taking into account individual goals and preferences.
- Collaborate with other agencies to provide effective support networks and promote independence.
- Manage crisis situations and lead safeguarding enquiries, maintaining high standards of practice.

Qualifications and Experience:
- Degree or Diploma in a relevant field (e.g., Social Work, Nursing, or Occupational Therapy).
- Registration with your professional body and competence at the new practitioner level of the Social Care Capabilities Framework.
- Post-qualification experience in social care, health, or related fields, including expertise in risk management and safeguarding enquiries.
