About the Role:
We are seeking a dedicated individual to join our team as an Adult Wellbeing Advocate. In this role, you will work closely with adults with care and support needs, their families, and carers to promote independence, wellbeing, and informed choices.
Key Responsibilities:
- Deliver person-centred care and support tailored to individual goals and preferences.
- Take a holistic approach, supporting families and carers to enhance the lives of those in their care.
- Build strong community links and partnerships to provide effective support networks.
- Conduct comprehensive assessments, create care plans, and review them regularly to meet personal outcomes.
- Manage crisis situations and lead safeguarding enquiries.
- Maintain high standards through personal reflection, supervision, and training.
Requirements:
- Degree or Diploma in a relevant field (e.g., Social Work, Nursing, or Occupational Therapy).
- Up-to-date registration with your professional body.
- Competence at the new practitioner level of the Social Care Capabilities Framework.
- Post-qualification experience in social care, health, or related fields.
- Expertise in risk management, safeguarding enquiries, and supporting individuals with medium to high-level complex needs.
- Proven ability to work collaboratively in multi-agency environments.
What We Offer:
- An estimated salary of £30,000 - £35,000 per annum, depending on experience.
