Community Cardiac Care Manager

We are looking for a skilled professional to fill the position of Community Cardiac Care Manager in our Surrey office.
About the Job:The successful candidate will be responsible for educating healthcare professionals, people with heart failure, and their caregivers on managing heart failure effectively. This includes teaching them to prevent short-term and long-term complications and acting as an advocate for patients.
- Responsible for developing effective joint working relationships with nursing and medical colleagues throughout partnership working.
- Provides advice and information on heart failure, that may be complex, to medical and nursing colleagues and the general public.
The salary for this position is £48,270 - £54,931 per annum inclusive of a 5% High Cost Area allowance.
The ideal candidate will have 5 years of nursing experience and 2 years of cardiac nursing experience. Good computer skills and counselling skills are also required.
