The NHS is seeking an experienced nurse to join their team as a Heart Failure Nurse Specialist in Surrey.

Job Description:

This role has been established to meet the needs of people with heart failure in the Surrey locality. The aim is to optimise the management of people with heart failure, reducing unnecessary hospital admissions and providing a link between GPs and specialist teams.

  • Responsible for communicating sensitive information and bad news to patients and carers in an empathetic and supportive manner.
  • Teaches patients and their carers to manage their heart failure to prevent short-term and long-term complications.
  • Acts as an advocate for patients, ensuring they receive understanding and facilities needed to manage their heart failure.

The ideal candidate will have 5 years of nursing experience and 2 years of cardiac nursing experience. A good computer skillset and counselling skills are also required.
