Job Title: Senior Practitioner

Thurrock Council is seeking a highly skilled Senior Practitioner to join our Adult Social Care Services team on a full-time permanent basis.

About the Role

We are looking for a dedicated and experienced professional to work in our Community Led Support (CLS) team. The successful candidate will be responsible for delivering high-quality social care services to vulnerable individuals, connecting them with their communities and promoting positive outcomes.

Key Responsibilities
  • Provide responsive social care support to individuals, families, and communities
  • Work collaboratively with colleagues and partners to deliver effective services
  • Develop and implement person-centred care plans to meet individual needs
  • Contribute to the development of the CLS team and Adult Social Care Services
Requirements

To be successful in this role, you will need:

  • A postgraduate qualification in social work or a related field
  • Experience of working in adult social care, preferably in a community-led support setting
  • Excellent communication and interpersonal skills
  • A commitment to safeguarding and promoting the well-being of vulnerable individuals
