Primary Care Network Administrator

Salary: £25,000 - £30,000 per annum.
We are looking for a skilled and enthusiastic individual to join our team as a Care Coordinator at a Primary Care Network. In this role, you will have the opportunity to make a real difference in the lives of our patients by supporting the delivery of high-quality women's health care.
Key Responsibilities- Developing Services: You will work closely with the Clinical Lead of the Women's Health Hub to develop and expand our services, identifying areas for improvement and implementing innovative solutions.
- Supporting PCN Clinical Leads: You will provide administrative support to PCN clinical leads, helping them to deliver projects and initiatives that benefit our patients.
- Managing Referrals: You will manage referrals from Linkworkers and Mental Health Practitioners, ensuring timely response and effective follow-up.
- ECDL or equivalent qualification
- Demonstrable commitment to professional and personal development
- Training in motivational coaching and interviewing or equivalent
