About the Job Opportunity

We are excited to offer a new opportunity for a Care Coordinator to join our team in North Liverpool. The successful candidate will play a key role in developing and expanding the Womens Health Hub and supporting the primary care network.

The estimated salary for this position is £25,000 - £30,000 per annum, depending on experience.

Main Responsibilities:

  • Manage appointments via EMIS Web
  • Organise efficient day-to-day running of the Womens Health Hub
  • Liaise with practices & health care professionals
  • Manage waiting lists
  • Manage stock requests
  • Service promotion across the PCN
  • Run EMIS searches
  • Use Excel programme to support LES claims
  • Work closely with the Clinical Lead of the Womens Health Hub (WHH) to facilitate advancement & promotion of the services
  • Manage Linkworker and Mental health practitioner referrals
  • Support PCN clinical leads with projects
  • Support Covid vaccination centre administration
  • Support covid vaccinators in housebound patients/Care homes when needed

Requirements

To be considered for this role, you must have:

  • ECDL or equivalent qualification
  • Demonstrable commitment to professional and personal development
  • Training in motivational coaching and interviewing or equivalent
  • Interest in the area of womens health services in primary care
  • Able to communicate effectively, both verbally and in writing, with people, their families, carers, partner agencies and stakeholders
