About the Role

We are recruiting for a Clinical Care Coordinator - Paediatrics to join our team at Acuity Care Group Limited. As a Clinical Care Coordinator, you will be responsible for coordinating the set-up of new client packages, including undertaking clinical assessments and developing care plans.

Responsibilities:

  • Coordinate the set-up of new client packages, including undertaking clinical assessments and developing care plans.
  • Develop and maintain strong relationships with clients, families, and healthcare professionals to ensure seamless care delivery.
  • Work collaboratively with Complex Care Managers (CCMs), ICBs, other professional bodies, and key team members in the organisation.

As a Clinical Care Coordinator, you will have the opportunity to develop your skills and knowledge in a dynamic and supportive environment. We offer a competitive salary of £35-40 per hour, depending on experience, and a range of benefits, including 25 days annual leave, pension contributions, car allowance, and statutory maternity/paternity leave.

Requirements:

  • Fully qualified paediatric nurse with valid UK driving license and access to own vehicle for travel between locations.
  • Experience in managing complex cases in different healthcare environments, showcasing versatility and comprehensive clinical expertise.
