Job Summary

We are seeking a dedicated and compassionate individual to join our team as a Care Facilitator at Burlington Primary Care. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.

About the Role

The successful candidate will be responsible for acting as the link between clinicians and clinical correspondence, scanning, coding, and matching externally received documentation using SystmOne. You will also provide and maintain a high standard of patient communication via telephone, letter, and SMS, delivered with a customer focus to the practice population.

Key Responsibilities

• Act as the front-line interface between patients and the Practice for the issue of prescriptions, adopting a professional and cordial manner at all times.

• Work alongside clinical and administrative colleagues to facilitate the provision of preventative healthcare to the practice population.

• Liaise with patients and care facilitate enquiries so as to deliver healthcare outcomes in line with Practice objectives and contractual responsibilities.

Person Specification

We are looking for an individual with excellent communication and interpersonal skills, who is a team player with a dash of independence. Ideally, you will have experience working with members of the public, using IT, and working in a healthcare setting. A GCSE grade A to C in English and Maths is also essential.
