Community Health Specialist

An exciting opportunity has arisen for a skilled and motivated nurse to join our Community Team for People with a Learning Disability in Hywel Dda University Health Board.
This role comes at a time of significant change in the healthcare sector, and we are seeking a candidate who can contribute to the design and delivery of a new service model that provides flexible and responsive services to adults and young people with learning disabilities.
As a Community Health Specialist, you will work proactively and creatively as part of a multidisciplinary team to deliver high-quality, evidence-based nursing care to individuals with learning disabilities and their families/carers.
You will have the opportunity to work closely with other colleagues in the multi-disciplinary team, providing specialist advice, support, and training to those on your caseload, their families, and carers, and to your colleagues across the Health Board.
Main Responsibilities- To have continuing professional accountability and responsibility for the provision of specialist health care, treatment, and advice for a defined caseload of clients who have learning disabilities with complex health needs, and their carers, in the community.
- To liaise and work in partnership with other agencies to deliver a flexible and responsive service that meets the needs of the local population of people with a learning disability and the needs of carers.
- To provide specialist advice, support, and training on a range of health issues to other agencies responsible for the provision of services/care to people with learning disabilities with the aim of reducing health inequalities for people with a learning disability.
