We are seeking an experienced Community Outreach Coordinator to join our team at CIH International Housing Group. In this role, you will work closely with clients who are homeless or at risk of becoming homeless in Bournemouth, Poole, and Christchurch, providing intensive one-to-one support and developing personalized support plans to help them achieve their goals.

The successful candidate will have a strong understanding of best practice in supporting vulnerable individuals, including those with complex needs such as substance misuse and mental health issues. As a key member of our team, you will work collaboratively with outreach workers, other agencies, and external partners to maximize opportunities for clients.

About the Position

This full-time position offers a competitive salary of £24,500 per year, depending on experience. You will work a 37-hour week, with a rolling rota that includes early, mid, and late shifts. This role requires staff to work some weekends and involves lone working in the community.

Main Responsibilities
  • Provide intensive one-to-one support to clients, focusing on addressing their immediate needs and long-term goals.
  • Develop and implement individualized support plans to address client needs.
  • Liaise with outreach workers and other agencies to build trust and rapport with clients.
  • Encourage clients to engage in services that improve their physical and mental health.
  • Help clients access benefits they are entitled to claim.
  • Work collaboratively with external partners to maximize opportunities for clients.
