Job Summary:

This is a fantastic opportunity for a skilled Advanced Nurse Practitioner to work autonomously, delivering care within their scope of practice to the entitled patient population. You will be part of a multi-disciplinary team, working closely with other healthcare professionals to provide high-quality patient care.

Key Responsibilities:
  • Deliver evidence-based nursing services to patients across Merseyside and surrounding areas
  • Work autonomously, making decisions within your scope of practice
  • Collaborate with other healthcare professionals to provide high-quality patient care
  • Develop, implement, and evaluate individual treatment plans for acute and non-acute conditions
  • Support the clinical team in the development of individual treatment plans for chronic disease patients
  • Identify, manage, and support patients at risk of developing long-term conditions
  • Provide routine nursing care to patients as required, in accordance with clinical-based evidence, NICE, and the NSF
  • Prescribe medication in accordance with PGD and within your own scope of practice
