We are seeking a dedicated and forward-thinking nurse to join our team at The Park Surgery in Driffield. As an Advanced Clinical Nurse Specialist, you will play a key role in delivering excellent patient care and working collaboratively with our primary care team.

Job Overview

This is a fantastic opportunity for an experienced nurse to take on a challenging and rewarding role, managing patients newly diagnosed and ongoing healthcare using practice template protocols/NICE Guidance and local guidelines.

Responsibilities
  • Manage patients newly diagnosed and ongoing healthcare using practice template protocols/NICE Guidance and local guidelines.
  • Undertake annual review of patients understanding and ability to self manage.
  • Refer to other healthcare professionals as necessary
  • Maintain a disease register.
  • Audit outcomes of care against national and local standards and initiate change as necessary.
  • Offer treatment room activities such as vaccinations and immunisations and cytology.

This role requires completion of a suitable accredited course; Initiates the process of diagnosis on patients with suspected diabetes.

Requirements
  • Experience of practice nursing
  • Experience of working in primary care and as part of a nursing team
  • Competent to carry out nursing duties as per job description
