About the Role

We are seeking a skilled Social Worker to join our team in supporting adults with complex social needs. As a Social Worker, you will work with vulnerable adults, their families, and carers to provide specialist advice and guidance.

Key responsibilities include:

  • Assessing and reviewing the needs of adults with complex social needs
  • Developing and implementing care plans to support vulnerable adults
  • Working with multi-disciplinary teams to provide specialist advice and guidance
  • Chairing meetings with clients, family members, and carers
  • Developing strong relationships with partner agencies and the local community

We are looking for a Social Worker who is passionate about supporting vulnerable adults and has a comprehensive understanding of current legislation, regulations, and guidance.
