Job Title: Care Coordinator

About the Role:

We are seeking an experienced Care Coordinator to join our dedicated team at Apollo Home Healthcare Ltd. As a skilled Care Coordinator, you will play a key role in the continuous delivery of care to service users.

Key Responsibilities:

  • Manage a caseload of care packages and understand the varied nature and fast-pace of the role.
  • Apply your gained communication and relationship building skills to effectively manage rota's for your care teams.
  • Respond efficiently to any day-to-day changes and act as the first point of contact for service users, their families and other healthcare professionals.

Requirements:

  • Experience in care coordination and management.
  • Excellent communication and relationship building skills.
  • Ability to work well under pressure and use your initiative.

  • Competitive salary between £28,290 - £33,290 pa.
