About the Role

We are seeking an experienced Care Coordinator to join our dedicated team in Albrighton, Wolverhampton. As a skilled Care Coordinator, you will be responsible for managing a caseload of care packages and understanding the varied nature and fast-pace of the role.

Key Responsibilities

  • Manage rota's for care teams and respond efficiently to any day-to-day changes
  • Act as the first point of contact for service users, their families and other healthcare professionals
  • Apply communication and relationship building skills to effectively manage care delivery

Requirements

  • Hold a Driving Licence and have access to a vehicle
  • Be committed to supporting the out of hours on call service
  • Have a competitive salary between £28,290 - £33,290 pa, including payment for on-call
