About the Role

We are seeking a dedicated and compassionate Care Coordinator to join our established team at C M Community Care Limited. As a Care Coordinator, you will play a vital role in delivering high-quality care services to our clients, ensuring their needs are met and exceeded.

In this full-time position, you will work alternative weekends and share out-of-hours phone duty as part of a wider team. Our shift pattern is fixed and continuous, allowing you to plan your life with confidence.

You will be part of a management team with vast experience, supported by innovative software that minimizes data input and maximizes time spent helping clients and staff.

Key Responsibilities
  • Deliver high-quality care services to clients, ensuring their needs are met and exceeded.
  • Work collaboratively with the management team to achieve team objectives.
  • Use innovative software to minimize data input and maximize time spent helping clients and staff.
  • Participate in out-of-hours phone duty as part of a wider team.
  • Contribute to the development and implementation of care plans.
  • Maintain accurate records and reports as required.
Requirements
  • A full UK driving license and car.
  • A smartphone.
  • A natural caring approach to life.
  • Excellent communication and teamwork skills.
  • Experience in a care setting.
