About the Role

We are seeking a skilled and compassionate Nurse to join our team as an Admissions and Discharge Coordinator in our Hospice-based multi-disciplinary Specialist Palliative Care In-Patient Unit. As a key member of our team, you will play a vital role in ensuring a seamless transition for patients as they arrive and are discharged from our unit.

Main Responsibilities
  • Coordinate patient admissions and discharges, ensuring a smooth transition and minimizing delays.
  • Work closely with the multi-disciplinary team to identify and address patient care needs, including complex care requirements.
  • Develop and maintain accurate and up-to-date patient records, providing essential information for discharge planning.
  • Liaise with external agencies and healthcare providers to ensure timely and effective discharge arrangements.
  • Support the development and implementation of care plans, including medication management and equipment provision.
  • Participate in educational programs and training initiatives to enhance professional development and contribute to the delivery of high-quality patient care.
Requirements
  • Current registration with the NMC as a Nurse.
  • Experience in specialist palliative care, preferably within a multi-disciplinary team setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Strong organizational and problem-solving skills, with a focus on delivering high-quality patient care.
  • Ability to work autonomously and as part of a team, with a commitment to ongoing professional development.
