About the Role

We are seeking a qualified Nurse to join our team as an Admission and Discharge Coordinator in our In-Patient Unit. As a key member of our Multi-Disciplinary Team, you will play a vital role in ensuring a smooth transition for patients as they arrive and when they are discharged.

Key Responsibilities
  • Work closely with the Multi-Disciplinary Team to provide holistic, person-centred care and support
  • Ensure a seamless transition for patients as they arrive and when they are discharged
  • Liaise with various stakeholders to coordinate patient care
