About the Role:

We are seeking a qualified Nurse to join our team as an Admission and Discharge Coordinator Nurse in our Specialist Palliative Care In-Patient Unit. As a key member of our multi-disciplinary team, you will play a vital role in ensuring a smooth transition for patients as they arrive and when they are discharged from the unit.

Key Responsibilities:

  1. Ensure a smooth transition for patients as they arrive and when they are discharged from the In-Patient Unit.
  2. Work alongside the multi-disciplinary team to provide holistic, person-centred care and support.
  3. Develop and maintain effective relationships with patients, families, and healthcare professionals.
  4. Contribute to the development and implementation of care plans that meet the individual needs of patients.
  5. Participate in ongoing education and training to enhance your skills and knowledge.

Requirements:

We are looking for a qualified Nurse with a passion for delivering high-quality care and support. You will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. A strong understanding of palliative care principles and practices is essential, as is the ability to work collaboratively as part of a multi-disciplinary team.
