Pharmacist for Patient Care

We are seeking a skilled Pharmacist for Patient Care to join our team at Lancaster Medical Practice. This is an exciting opportunity to work in a multidisciplinary environment, providing clinical medicines advice and supporting patients with their medication needs.
The successful candidate will have excellent communication skills, ability to plan and manage medicine optimization issues, and good general computer skills. They will also be able to demonstrate a conscientious approach and commitment to working in an adaptable and flexible manner.
Key Responsibilities:
- Work as part of a multidisciplinary team to provide clinical reviews and desk-based activities
- Support and work alongside a team of pharmacists in general practice
- Help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation
- Provide expert advice on clinical medicines and address public and social care needs of patients
Requirements:
- Mandatory registration with the General Pharmaceutical Council
- Masters Degree in Pharmacy (MPharm) or equivalent
- Membership of the Royal Pharmaceutical Society
- Demonstrate excellent administration and organisational skills
- An excellent standard of grammar and spelling in the English language
Estimated Salary: £35,000 - £45,000 per annum
