Care Manager Specialist

We are seeking an experienced Paediatric Nurse to fill a challenging role as a Care Manager Specialist. In this position, you will be responsible for delivering high-quality care packages commissioned by healthcare authorities.

The successful candidate will have the opportunity to develop and implement individualised care plans, working closely with a multidisciplinary team to provide comprehensive support.

Key Responsibilities:
  • Designing and implementing tailored care strategies specific to each client's needs
  • Effective communication and collaboration with the multidisciplinary team to achieve optimal patient outcomes
  • Ongoing monitoring, assessment, and evaluation of clients' physical and psychological needs within your caseload
  • BUILDING STRONG RELATIONSHIPS WITH CLIENTS AND FAMILY MEMBERS
  • Collaboration with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke care packages
Requirements:
  • RSCN with a minimum of 2 years' experience in the UK and an active NMC PIN
  • A supportive management style that fosters growth and development among team members
  • Experience with tracheostomy care and ventilators
  • Advanced communication skills, both verbal and written, to liaise effectively with patients, families, and the multidisciplinary team
  • A full UK driving licence is essential
