Job Overview

We are seeking an experienced and skilled Complex Care Manager to join our team at Acuity Care Group Limited. As a key member of our Bespoke and Team Brain Injury Support teams, you will be responsible for managing support packages with varying requirements.

The ideal candidate will have a genuine passion for delivering high standards of care and be committed to making a positive impact on the lives of those we support. You will work closely with our Operations Manager and other internal departments to ensure seamless communication and collaboration.

Key Responsibilities:

  • Conduct thorough assessments to understand individual needs and preferences
  • Develop and implement personalised care plans addressing medical, emotional, and social requirements
  • Regularly review and adjust care plans to ensure relevance and effectiveness
