Senior District Nurse Leader

We are seeking a dynamic and experienced individual to lead our Community Nursing Teams within the East North Integrated Community Nursing and Therapy Hub. As a Senior District Nurse, you will be responsible for providing senior support and clinical oversight to our teams, ensuring the delivery of high-quality, evidence-based nursing care to patients in various community settings.

Main Responsibilities- To lead and manage Community Nursing Teams within the locality hub, overseeing the assessment, management, and evaluation of evidence-based nursing care.
- Line management of District Nurses/Team Leads, responsible for day-to-day management of teams within the locality hub.
- To promote effective teamwork within the wider primary health care team, delegating activities appropriately to benefit patient care and service delivery.
- To monitor the quality of care provision and identify areas for service development.
- To have overarching responsibility for the performance of teams and individuals within the hub.
- To act as a clinical role model and support care delivery for complex patients on the Community Nursing caseloads.
- To work in partnership with key stakeholders to ensure high-quality, integrated, and seamless care is delivered.
- To act as a key point of contact for GPs and other general practice staff at a hub level.
