Job Overview

We are seeking a dynamic and experienced individual to lead our Community Nursing Teams within the East North Integrated Community Nursing and Therapy Hub. As a Senior District Nurse, you will have the opportunity to provide senior support and clinical oversight, working closely with our teams of highly motivated professionals.

This is an exciting opportunity for a skilled and enthusiastic nurse to take on a leadership role, working in partnership with key stakeholders to ensure high-quality, integrated, and seamless care is delivered to our patients.

Main Responsibilities
  1. To lead and manage Community Nursing Teams within the locality hub, overseeing the assessment, management, and evaluation of evidence-based nursing care in various community settings.
  2. Line management of District Nurses/Team Leads, responsible for day-to-day management of teams within the locality hub.
  3. To promote effective teamwork within the wider primary healthcare team, delegating activities appropriately to benefit patient care and service delivery.
  4. To monitor the quality of care provision, identifying and promoting areas for service development.
  5. To have overarching responsibility for the performance of teams and individuals within the hub.
  6. To act as a clinical role model, supporting care delivery for complex patients on the Community Nursing caseloads.
  7. To work in partnership with the Community Service Matron and Clinical Leads to deliver high-quality care that meets commissioner requirements.
  8. To act as a key point of contact for GPs and other general practice staff at a hub level.
