Job Title: Senior Community Nurse Leader

We are seeking an experienced and skilled Senior Community Nurse Leader to join our team at Leicestershire Partnership NHS Trust. As a key member of our community nursing team, you will be responsible for leading and managing community nursing teams within a locality hub, providing clinical oversight and ensuring high-quality care for patients in various community settings.

Main Responsibilities:
  • Lead and manage community nursing teams, overseeing the assessment, management, and evaluation of evidence-based nursing care.
  • Line manage District Nurses/Team Leads, responsible for day-to-day management of teams within a locality hub.
  • Promote effective teamwork within the primary healthcare team, delegating activities to benefit patient care and service delivery.
  • Monitor quality of care provision, identifying areas for service development and promoting best practices.
  • Oversee team performance, ensuring high-quality care delivery and meeting commissioner requirements.
  • Act as a clinical role model, supporting care delivery for complex patients on community nursing caseloads.
  • Collaborate with key stakeholders to ensure seamless care delivery, working in partnership with GPs and other general practice staff.
