The post holder will be responsible for providing a single point of access for all referrals into Adult Mental Health services. This involves reviewing referrals to identify gaps in the referral process and strengthening existing joint partnership systems to improve early and appropriate referrals.
Main DutiesThe successful candidate will be responsible for:
- Triage all referral information, making autonomous clinical judgments to prioritize and allocate referrals to the appropriate professional within our service or suggest alternatives to the referrer.
- Collate and analyze statistical data from the current database to facilitate the process.
- Conduct holistic assessments and formulate risk assessments.
- Provide brief interventions during telephone calls from external agencies and the general public.
This is an exciting opportunity to join a developing and expanding team to improve access and response to mental health services. The post is for a dedicated Single Point of Access Professional, open to qualified mental health professionals.
The successful candidate will be part of a team that encourages development and support, with a focus on delivering high-quality care and support to individuals experiencing mental health concerns.
RequirementsThe ideal candidate will have:
- RMN registration, occupational therapy, or social work qualifications.
- Substantial post-qualification experience as a Band 5/6 or equivalent working within the field of acute adult/community mental health.
- Experience of teaching and training other health and/or social care professionals and designing educational inputs (desirable).
