Job Summary

We are seeking a highly skilled and motivated Community Psychiatric Nurse to join our team in Port Talbot. As a key member of the Older Persons Community Health Team, you will be responsible for delivering high-quality care to patients with severe and enduring mental health problems.

Main Responsibilities

You will be required to engage with patients living in the community primarily over the age of 65, presenting with symptoms suggestive of an organic illness (dementia, cognitive impairment) and other functional illnesses including depression and anxiety. Your duties will include arranging assessments, implementing and evaluating care, and ensuring involvement of the Multi-Disciplinary Team. You will work closely with the Consultant Psychiatrist, Social Services, and third sector and voluntary agencies to ensure patients receive the right care and interventions.

Requirements

  • Professional Qualification - RMN
  • Experience in Community Nursing Older Persons
  • Able to demonstrate clinical expertise for band 6
  • Knowledge of clinical risk assessments
  • Knowledge of Safeguarding Adults/Children including the Mental Health Act and Mental Capacity Act and Deprivation of Liberty Safeguards

Desirable

  • Teaching qualification (or equivalent experience)
  • Recognised management qualification
  • Understanding of wider nursing and health care related agenda
