About the Role
We are seeking a highly motivated and experienced Registered Mental Health Nurse to join our Community Mental Health Team as a Care Coordinator. The successful candidate will work autonomously and as part of a multidisciplinary team to deliver high-quality community mental health services under the framework of the Care Programme Approach and the Mental Health Measure (2010) Wales.
Main Responsibilities
- Assess, implement, and evaluate agreed care and treatment plans for individuals on the caseload.
- Work autonomously but under the indirect supervision of a Band 7 registered nurse/team manager.
- Collaborate with unregistered Band 3 healthcare assistants, other health colleagues, and colleagues from the Local Authority to deliver a supportive service.
Requirements
- Registered Mental Health Nurse qualification.
- Postgraduate qualification or evidence of working towards the same.
- Experience of working at Band 5 in a multidisciplinary team framework in Acute Adult Mental Health Services.
- Ability to communicate and engage effectively with service users suffering from severe mental health problems.
