About the Role

We are seeking a highly motivated and experienced Registered Mental Health Nurse to join our Community Mental Health Team as a Care Coordinator. The successful candidate will work as part of a multidisciplinary team to provide high-quality care and support to individuals with severe mental health problems.

Key Responsibilities
  • Assess, plan, and evaluate care plans to meet the individual needs of service users.
  • Work autonomously with an ability to communicate and engage effectively with service users.
  • Collaborate with other agencies and services to provide a comprehensive support network.
  • Manage time, prioritize work, and utilize problem-solving strategies to meet the needs of service users and the service as a whole.
  • Assess and respond to risk, using evidence-based literature and guidelines.

Requirements
  • Registered Mental Health Nurse qualification.
  • Postgraduate qualification or evidence of working towards the same.
  • Experience of working at Band 5 in a multidisciplinary team framework in Acute Adult Mental Health Services.
  • Ability to communicate and engage effectively with service users.
  • Autonomous working and problem-solving skills.
Desirable
  • Relevant Nursing Degree.
  • Diploma in relevant Nursing/Community Studies.
  • Experience of working with family and carers.
  • Ability to speak Welsh.
