We are seeking a highly motivated and experienced Registered Mental Health Nurse to join our Community Mental Health Team as a Care Coordinator. The successful candidate will work as part of a multidisciplinary team to provide high-quality care and support to individuals with severe mental health problems.
Key Responsibilities- Assess, plan, and evaluate care plans to meet individual needs of service users.
- Work autonomously with an ability to communicate and engage effectively with service users.
- Collaborate with other agencies and services to provide a comprehensive support network.
- Manage time, prioritize work, and utilize problem-solving strategies to meet the needs of service users and the service as a whole.
- Assess and respond to risk using evidence-based literature and guidelines.
- Registered Mental Health Nurse with postgraduate qualification or evidence of working towards the same.
- Experience of working at Band 5 in a multidisciplinary team framework in Acute Adult Mental Health Services.
- Ability to travel between sites in a timely manner.
- Excellent communication and interpersonal skills.
