About the Role

We are seeking a dedicated and compassionate Care and Support Professional to join our team at Active Care Group. As a Support Worker, you will play a vital role in delivering high-quality, person-centred care to our patients.

Key Responsibilities
  • Assist nursing staff in providing individualised care to patients, relatives, and carers.
  • Work collaboratively with the team to support patients in achieving their planned outcomes.
  • Build strong relationships with patients, fostering open and two-way communication.
  • Support patients in their rehabilitation programme, promoting independence and well-being.
  • Establish effective methods of communication with patients, addressing any barriers that may arise.
  • Assist with activities of daily living and personal care, ensuring patients' dignity and comfort.
  • Communicate any issues or concerns to the clinical team, ensuring seamless care delivery.
  • Accurately record and report work activities, maintaining high standards of documentation.
