Care Support Team Leader

Job Summary:
We are seeking a highly skilled and experienced Care Support Team Leader to join our team at Care Support. As a key member of our leadership team, you will be responsible for overseeing the provision of care and support services at our Extra Care Scheme, ensuring that our customers receive the highest level of care and support.
Key Responsibilities:
- Leadership and Management: Provide leadership and guidance to care and support staff, ensuring that they have the necessary skills and knowledge to provide high-quality care and support services.
- Customer Care: Ensure that our customers receive person-centered care and support, respecting their dignity, privacy, choice, independence, fulfillment, and rights.
- Communication and Collaboration: Develop and maintain effective working relationships with staff, customers, and external partners, including housing, health, and social care professionals.
- Policy and Procedure: Adhere to all Care Support policies and procedures, ensuring that our services are delivered in accordance with regulatory requirements.
- Staff Management: Manage the staff rota, ensuring that sufficient staff are available to meet the needs of our customers.
- Needs Assessment and Support Planning: Participate in needs assessments, risk assessments, and support planning, identifying customers' needs, goals, and aspirations.
- Reabling Care and Support: Deliver effective, outcome-based reabling care and support, monitoring and measuring progress towards outcomes and reabling goals.
- Risk Management: Ensure a safe living environment for our customers, respecting their choice and rights, and contributing to the protection of customers from abuse.
- Teamwork and Collaboration: Work as part of a team, contributing to the delivery of high-quality care and support services, and being able to work alone when required.
- Professional Development: Attend supervision, training, and staff meetings, as required, and be willing to undertake any other tasks specified by line management.
